E F Ruth Miller PhD
Pharmacist. Researcher. The person behind Build. Transform. Last.
I'm a pharmacist, researcher, and healthcare service designer with more than three decades of experience. I created BTL because I kept returning to the same question: how do we design, deliver and evaluate healthcare services so they are more likely to succeed, sustain beneficial outcomes, and be capable of scale? Build. Transform. Last. is the answer I arrived at.
How BTL came to exist
It didn't start with a philosophy. It started with a question I couldn't leave alone.
Throughout my career across sectors, services and organisations, I watched the same pattern repeat. Some services flourished while others struggled. Some innovations scaled beyond their original setting while others never moved beyond pilot status. And the difference couldn't always be explained by funding, structure, or effort. Something else was at work.
The people I worked alongside were, without exception, capable and committed. The problem was never them. It was the way services were being designed.
"Not failure of effort, not failure of intention — but failure of architecture. The evidence was missing because the need to build it in from the start was never considered."
BTL emerged from that realisation — a practical philosophy built on eight principles that I have found most consistently contribute to services that succeed. Not by chance, but by deliberate design.
The Evidence Future Ready Model (EFRM) is the organisational capability model derived from that philosophy. It is how BTL becomes operational in practice.
Much of the thinking was shaped within Northern Ireland's Health and Social Care system — one of the most complex, resource-pressured healthcare environments in the world. That is not a limitation; it is a proof of concept. The principles of BTL are designed to be applicable across healthcare settings, organisations and systems internationally.

What drives this
Making a difference. Not incrementally. At scale.
BTL and the Evidence Future Ready Model are my legacy. Built from thirty years of clinical practice, research, and the hard-won understanding that good intentions are never enough on their own.
Healthcare deserves services designed to outlast the people who created them, the projects that funded them, and the political cycles that shaped them.
"That belief is in every page of this book. It is in every principle of BTL. And it is what I bring when I work with organisations who are ready to build something that lasts."
Read the book. Apply the philosophy.
Build. Transform. Last. is out now.
